Weeks 1–2
Pain
Calm the knee down.
Hands-on care and guided movement control swelling and pain in the knee, starting your very first visit.
What We Treat
The stiffness when you stand up from the couch. The twinge that decides whether today’s a good day. It’s fixable, and it starts with a cause-first evaluation, not a guess.

Why It Happens
ACL tears usually happen in a split second: a sudden pivot, a sharp deceleration, or an awkward landing, often during sports like soccer, basketball, or skiing, where the knee twists while the foot stays planted on the ground. Direct contact tears happen too, from a hit to the side of the knee. But weak or imbalanced hip and thigh muscles, and landing mechanics that put extra twisting force through the knee, often set the injury up long before the moment it actually happens.
None of that gets fixed by rest alone. It takes a real, cause-first evaluation of your knee’s stability, strength, and movement patterns, whether surgery is part of your path or not, to build a plan that actually restores trust in the knee instead of just waiting for swelling to go down.
The Honest Truth
Nobody regrets starting too early. Every week, someone in our clinic says they regret waiting.
How We Fix It
Every plan has an endpoint. Here’s what the four phases look like for a torn ACL.
Weeks 1–2
Pain
Hands-on care and guided movement control swelling and pain in the knee, starting your very first visit.
Weeks 2–6
Prime
Targeted mobility work restores full knee bend and extension without the compensations that slow real recovery.
Weeks 6–10
Perform
Progressive strengthening for your quad, hamstring, and hip rebuilds the stability your knee lost.
Weeks 10+
Prevent
Cutting, pivoting, and landing mechanics get tested under real sport-speed load before you’re cleared to go all-in: that’s the endpoint, not just another appointment.

Questions, Answered
Not always, and even when surgery is the right call, physical therapy usually starts either way. If you think of yourself as someone who plays through pain, that instinct got you this far, but with a torn ACL it’s exactly the instinct that costs you months later. We start with a real evaluation of your knee’s stability and strength to help you and your surgeon decide what’s next, then build the plan around it. You don’t need every answer before your first visit, you need a real look at the knee.
Yes, in a carefully progressed way, movement is what rebuilds a knee that trusts itself again. Staying still too long lets the muscles around your knee weaken further, which makes the knee feel even less stable. The key is dosing the right movement and load for where your knee actually is, which is exactly what a physical therapist sorts out at your evaluation, then progresses safely as you heal.
Most patients feel meaningful change within 4–6 visits. A full plan for an ACL tear runs through all four phases, Pain, Prime, Perform, and Prevent. The exact timeline depends on whether you’ve had surgery and your sport or activity goals, but you’ll always know which phase you’re in and what’s left, not an open-ended string of appointments.
